About The Position

Emblem Hospice is hiring a full-time Hospice RN Case Manager to provide services across East Mesa and Apache Junction. The role follows a Monday-Friday schedule and delivers hospice care to patients in their homes and communities. The nurse serves as a trusted case manager, guides families through care decisions, and provides comfort, dignity, and compassionate support. Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona.
• Minimum of 2 years of nursing experience.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.

Nice To Haves

• Hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, but not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising them as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
