About The Position

Community Care is hiring a full-time RN Case Manager for its Family Care Program team. The position is based out of the West Bend office and serves Washington and Dodge Counties. The role supports older adults and adults with physical and intellectual disabilities in living as independently as possible in the community. The RN Case Manager collaborates with a team and works with members, families, other supports, and community partners to coordinate personalized care. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live independently in the community. It coordinates and delivers supportive services to more than 13,000 Wisconsin residents and has local offices supporting the 23 counties it serves. The organization works with members to develop care plans addressing health and social needs and is committed to compassionate, personalized care that supports healthy, independent living. This position is scheduled Monday through Friday, 8:00 AM to 4:30 PM. A $8,000 sign-on bonus is offered.

Requirements

Associate degree in Nursing.
Wisconsin Licensed Registered Nurse.
Eligibility for long-term care functional screener certification.
Wisconsin driver's license.
Two years of recent nursing experience with older adults or people with physical or intellectual disabilities.

Nice To Haves

Bachelor's degree in Nursing preferred.
Two years of work in community nursing preferred.

Responsibilities

Complete discipline-specific initial and ongoing assessments.
Conduct home visits and home assessments as needed.
Consult with families and other supports to obtain additional information as needed.
Develop and maintain care plans that address members' long-term care and personal outcomes, progress toward previously identified outcomes, strengths, risks, and natural and formal supports.
Follow up on services to ensure their quality, coordination, and effectiveness.
