About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager assesses patients’ needs, implements and recommends revisions to individualized plans of care, coordinates care with patients, caregivers, physicians, healthcare professionals, and the Inter-Disciplinary Group, and supports compliance with applicable laws, regulations, and agency policies. The position includes assigned on-call responsibilities and timely documentation.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state Registered Nurse license.
Current driver’s license and reliable transportation.
Provide ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience in a healthcare or clinical setting is preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement the individualized plan of care and recommend revisions as necessary.
Coordinate care among patients, caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings; coordinate and participate in care planning and prioritization of needs; and collaborate with the group to promote coordinated patient care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
