About The Position

The RN Case Manager is responsible for performing visits and coordinating client care. This role serves as the primary point of contact for all disciplines involved in patient care and manages the frequency of visits for each episode of care. The RN Case Manager collaborates with physicians and the office, providing detailed updates on patient care. Key responsibilities include accurate OASIS collection, maintaining up-to-date medication profiles, timely reporting of lab values to physicians, and participating in weekly case conferences and monthly case manager meetings. All patient care must be delivered with respect for patient rights, dignity, and expressed needs. The role involves assessing patients' physical, psychological, and social needs, managing pain through appropriate techniques, and meticulously documenting patient history, symptoms, assessments, tests, and care according to established guidelines. A critical part of the job is reviewing patient history, diagnostics, and lab data, and promptly reporting any abnormalities.

Requirements

  • Current, unencumbered RN license in Idaho
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments

Responsibilities

  • Perform visits and complete coordination of client care.

  • Consult with the physician and the office regarding patient care.
  • Perform accurate OASIS collection.
  • Ensure the medication profile remains current.
  • Ensure lab values have been reported to the physician timely.
  • Attend weekly case conference and monthly case manager meetings.
  • Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needs.
  • Assess the patient’s physical, psychological, and social needs.
  • Assess pain and implement appropriate pain management techniques.
  • Document patient history, symptoms, assessments, tests, and care according to guidelines.
  • Review patient history, diagnostics, and lab data, and report any abnormalities.
