They are seeking a qualified Hospice and Home Health RN Case Manager who shares their values and commitment to excellent care. Requirements

• Licensed as a Registered Nurse by the State of Utah
• Current Utah driver's license and automobile insurance
• CPR certification
• A minimum of 1 year nursing experience is required
• Associate degree
• Registered Nurse (RN) license or certification
• Good documentation skills
• Enjoy caring for patients in a home setting

Nice To Haves

• Hospice and home health experience is strongly preferred but not required

Responsibilities

• Provide skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
• Create patient-centered goals with patients, family members, and providers.
• Assess, supervise, and direct patient care.
• Develop plans of treatment.
• Ensure chart compliance with state regulations.
• Coordinate patient care with physicians and other providers.
