About The Position

The RN Case Manager provides leadership that facilitates professional nursing care for patients’ comfort and well-being. Using the nursing process—assessment, planning, implementation, and evaluation—the nurse provides, delegates, supervises, and documents patient and family care and teaching, and implements medically prescribed interventions. The position is with CarePartners Health Services, which provides home health, hospice, rehabilitation, and other post-acute care services in western North Carolina.

Requirements

• Associate's degree or Bachelor of Science in Nursing.
• Current registered nurse licensure with the North Carolina Board of Nursing; an eNLC license may apply, and the licensee should confirm with NCBON.
• Registered Nurse licensure must be obtained within 30 days of the employment start date.
• BLS Healthcare Provider certification.
• One year of nursing experience.

Responsibilities

• Facilitate the delivery of professional nursing care for patients’ comfort and well-being.
• Use the nursing process to assess, plan, implement, and evaluate patient care.
• Provide, delegate, supervise, and document patient and family care and teaching.
• Implement medically prescribed interventions.
