About The Position

The RN Case Manager is responsible for performing visits and completing the coordination of client care. This role serves as the point of contact for all disciplines involved in patient care and oversees the frequency of visits for the episode. The RN Case Manager consults with the physician and office staff regarding patient care, performs accurate OASIS collection, maintains current medication profiles, ensures timely reporting of lab values to the physician, and attends weekly case conferences and monthly case manager meetings. All patient care must be provided in a manner that reflects respect for patient rights, dignity, and expressed needs. This includes assessing the patient’s physical, psychological, and social needs, assessing pain and implementing appropriate pain management techniques, and documenting patient history, symptoms, assessments, tests, and care according to guidelines. The RN Case Manager also reviews patient history, diagnostics, and lab data, reporting any abnormalities.

Requirements

  • Current, unencumbered RN license in Idaho
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments
