Home Health / Hospice RN Case Manager

We are seeking a qualified Hospice and Home Health RN Case Manager who shares our values and commitment to excellent patient care. Requirements
- Licensed as a Registered Nurse by the State of Utah
- Current Utah driver's license and automobile insurance
- CPR certification
- A minimum of 1 year nursing experience is required
- Associate degree
- Nursing: 1 year experience
- Registered Nurse (RN) license or certification
Nice To Haves
- Hospice and home health experience is strongly preferred but not required
- Good documentation skills
- Enjoy caring for patients in a home setting
Responsibilities
- Provide skilled nursing care, including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching.
- Conduct hospice assessments and other skilled care.
- Create patient-centered goals with patients, family members, and providers.
- Assess, supervise, and direct patient care.
- Develop plans of treatment.
- Ensure chart compliance with state regulations.
- Coordinate patient care with physicians and other providers.
