About The Position

Careforth is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones. The role involves assessing needs, developing individualized care plans, conducting home visits, and helping ensure quality consumer care. The position offers a flexible schedule, a remote-first culture, and no night shifts, weekends, or on-call duties.

Requirements

Active Registered Nurse (RN) license in the state of employment.
At least two years of recent experience providing direct care to elders or adults with disabilities.
Ability to travel, usually by automobile, within the assigned geographic area.
Valid driver’s license and automobile insurance.
Experience in case management, care planning, and assessment in a healthcare setting, including cases involving eldercare, people with disabilities, or other complex medical needs.
Proficiency with common software and web-based applications, such as MS Office and case management systems.
Additional state-specific qualifications may be required.

Responsibilities

Conduct home visits in the assigned territory to assess home safety, gather updates on health status, and provide clinical coaching and support, including medication reviews.
Provide ongoing coaching and support to caregivers through an app, following established care plans; refer caregivers to resources and reinforce best practices.
Develop trusting relationships with caregivers to influence change and reduce caregiver burnout.
Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards.
Perform other projects and duties as assigned.
