About The Position

Navitas Healthcare, LLC is seeking an RN - Case Manager for an exciting Travel opportunity in Hyannis, MA. This role involves leading and overseeing daily Case Management department operations, supervising clinical resource management and discharge planning, and ensuring compliance with CMS regulations and organizational standards. The position also includes developing and implementing process improvement initiatives, collaborating with interdisciplinary teams to improve patient outcomes and care coordination, and monitoring resource utilization, length of stay, and quality metrics. Additionally, the role involves assisting in the development and presentation of continuing education programs, providing leadership and mentorship to case management staff, and participating in rotating weekend and holiday on-call coverage. Accurate documentation and reporting within the Epic EMR are also key responsibilities.

Requirements

  • Bachelor of Science in Nursing (BSN) required
  • Current Massachusetts RN License required
  • Minimum 5 years of Acute Care Case Management experience required
  • Minimum 2 years of Process Improvement experience required
  • Strong knowledge of CMS regulations
  • Epic EMR experience required
  • Excellent analytical, organizational, interpersonal, and writing skills
  • Strong computer proficiency and communication skills

Nice To Haves

  • Master’s Degree preferred
  • Management or supervisory experience preferred
  • Certified Case Manager (CCM) certification preferred
  • Clinical Documentation Integrity certification preferred
