About The Position

In collaboration with the patient and family, physicians, and the interdisciplinary team, the RN Case Manager facilitates the patient’s progress through an acute episode of care in an efficient and cost-effective manner. The role monitors level of care; initiates evidence-based protocols and order sets according to the patient’s admission diagnosis; influences the progression of care; facilitates navigation through the acute care episode; and proposes an appropriate transition plan within 48 hours of admission. The Case Manager advocates for the patient and family throughout the entire acute episode of care.

Requirements

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing (BSN).
• At least 1 year of experience as a Registered Nurse in an acute care setting.
• PA Registered Nurse License or Multi State Compact Registered Nurse License.

Nice To Haves

• At least 2 years of experience as a Case Manager in a hospital setting.
• Certified Case Manager credential.
