RN Case Manager - Hospice
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role includes patient assessments, care planning, referrals to other hospice disciplines when needed, maintaining accurate patient progress records, recording pain and symptom management outcomes, and teaching patients, families, and caregivers appropriate care techniques. The RN Case Manager regularly communicates with the clinical manager and care team and performs on-call responsibilities and services as assigned. Other duties may be assigned.
Requirements
- Current RN license specific to the state or states assigned for work.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of nursing experience.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents the rationale for not doing so.
- Develops, prepares, and maintains individualized patient care progress records accurately, in a timely manner, and according to care center policies.
- Records pain and symptom management changes and outcomes as appropriate.
- Teaches patients, families, and caregivers appropriate patient care techniques.
- Regularly communicates with the clinical manager and care team.
- Performs on-call responsibilities and provides on-call services to patients and families as assigned.
- Performs other duties as assigned.
