About The Position

The Hospital Case Manager RN is responsible for high-quality, cross-continuum care coordination. The role works with physicians and APPs, peers, community colleagues, and others to coordinate care across the healthcare continuum, identify opportunities to improve patient care and services, improve population health, and support collaborative practices that reflect MultiCare Health System’s commitment to patient-centered care and community engagement. This is a 1.0 FTE day-shift position, scheduled for four 10-hour shifts with rotating weekends and holidays. The role is based at MultiCare Covington Medical Center, a four-story, 58-bed hub serving southeast King County with inpatient and outpatient surgical care, a birth center, primary and specialty care, urgent care, a 24-hour Emergency Department, pharmacy, lab, and imaging services.

Requirements

Bachelor’s degree in Nursing, or obtain one within five years of the initial date of hire. An RN with 15 or more years of service, or an internal candidate hired before January 1, 2021, may be considered in satisfaction of this requirement in lieu of an education agreement.
Current Registered Nurse license in Washington State or Multistate License endorsement (MLS).
Four years of recent hospital case management experience.
Five years of recent clinical hospital or ambulatory experience.

Nice To Haves

Advanced degree in Nursing (MSN, PhD, DNP, or ARNP) preferred.
Healthcare financial and reimbursement knowledge preferred.
Leadership and emergency services experience is a plus.

Responsibilities

Ensure patients and families experience medically appropriate transitions of care as an integral part of care delivery.
Advocate for patients and families, including transitions to a lower level of care when inpatient care is no longer needed.
Partner with and support physicians and APPs in patient care activities and care transitions.
Use critical thinking in all interactions.
Be accountable for transitions of care and care across the continuum.
