RN Case Manager Hospice 10K Sign-on Bonus - Marlborough, MA
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role includes patient assessments and care planning, referrals to other hospice disciplines when needed, maintaining accurate and timely patient care records, tracking pain and symptom management outcomes, and teaching patients, families, and caregivers appropriate care techniques. The RN Case Manager regularly communicates with the clinical manager and care team and performs on-call responsibilities and services as assigned. Other duties may be assigned. The position offers a $10,000 sign-on bonus for external candidates. Salary ranges from $91,540 to $137,310 annually based on full-time employment. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- Current RN license specific to the state or states assigned for work.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of nursing experience.
- Must pass a drug test before beginning employment.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately, in a timely manner, and according to care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
