RN Case Manager Hospice $10K Sign-on Bonus - Boston, MA
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services that help people receive care at home. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, working with the interdisciplinary care team, and developing plans that address each patient’s needs. Patient coverage territories include Boston, Roxbury, West Roxbury, and Milton, Massachusetts. The position involves patient and caregiver education, accurate and timely clinical documentation, communication with the clinical manager and care team, and on-call duties as assigned. The role offers a salary range of $91,540 to $137,310 annually based on full-time employment. External candidates are eligible for a $10,000 sign-on bonus, subject to applicable terms. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.
Requirements
- Current RN license specific to the state or states where the employee is assigned to work.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of nursing experience.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not making a referral.
- Develop, prepare, and maintain individualized patient care progress records accurately and promptly, in accordance with care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
