About The Position

Amedisys is hiring a full-time, day-shift RN case manager to serve patients in Chester and Montgomery Counties. The role provides hospice care in patients’ homes and involves coordinating with patients, families, caregivers, and the care team. The position includes a $5,000 sign-on bonus and an annual salary range of $90,000–$93,000. On-call responsibilities and services are performed as assigned.

Requirements

• At least one year of nursing experience.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement plans of care.
• Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and promptly in accordance with care center policies.
• Record changes and outcomes related to pain and symptom management as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
