About The Position

Sage Group HealthCare Staffing is seeking a Hospice RN Case Manager for a contract position in Barre, VT. This role is open to both local and traveler candidates. The position involves screening, evaluating, re-evaluating, and treating patients, as well as coordinating and communicating treatment activities with the interdisciplinary team and the patient. A key responsibility is developing and periodically reviewing a care plan in collaboration with the patient's physician, patient, and their family or primary caregivers. The RN Case Manager will coordinate the patient's care plan through interdisciplinary communication and case conferences, and advise other disciplines of discharges in a timely manner. Ongoing assessment of the patient's clinical, functional, and emotional status is required to identify problems and progress towards goals. Significant changes must be communicated to the patient's physician and colleagues. The role also includes documenting evaluations, treatment goals, and objectives, and teaching the patient and caregiver self-care techniques, medication, diet, and other instructions as ordered by the physician, utilizing opportunities for health counseling.

Requirements

  • Registered Nurse: 2 years - Required
  • Hospice in home setting RN experience: 2 years – Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State/s applying or Compact - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • McKesson/NetSmart EMR experience - Preferred
  • BSN or an equivalent – Highly preferred

Responsibilities

  • Screen, evaluate, re-evaluate, and treat patients.
  • Coordinate and communicate treatment activities with the interdisciplinary team members and the patient.
  • Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
  • Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
  • Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
  • Advise other disciplines of discharges in a timely manner.
  • Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
  • Communicate significant changes to the patient's physician and colleagues.
  • Document evaluations, treatment goals, and objectives.
  • Teach the patient and caregiver self-care techniques as appropriate.
  • Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.
