About The Position

Amedisys is hiring a full-time RN case manager to provide hospice care in Wyoming, Susquehanna, and a portion of Wayne County. The role focuses on assessing patients, coordinating and implementing individualized plans of care with the care team, maintaining accurate clinical records, managing symptoms, and teaching patients, families, and caregivers. The position includes assigned on-call responsibilities. The annual salary range is $88,000–$95,000, and the posting offers a $10,000 sign-on bonus.

Requirements

• One year of nursing experience.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and on time, according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
