RN Care Manager-Cross Care Continuum
About The Position
The RN Care Manager provides comprehensive direct patient continuum-of-care and case-management services. The role coordinates referrals to health and social welfare resources and supports discharge planning in conjunction with the Continuum Care Manager or Care Navigator. As an integral member of a collaborative, interdisciplinary team, the Care Manager supports nursing and other clinical and non-clinical ancillary services in assuring continuity of care and maintaining quality service delivery. The role also serves as a patient advocate. This is a full-time position based in Evanston, Illinois, with a 40-hour workweek, Monday through Friday from 8:30 a.m. to 5:00 p.m. Rotating weekend work and coverage for one winter and one summer holiday are required.
Requirements
- LSW or LCSW license required.
- Master’s degree from an accredited school of social work required.
- CPR BLS certification from the American Heart Association (AHA) required.
- Two years of post-graduate experience preferred; healthcare and/or mental health experience preferred.
- Experience with an Electronic Medical Record (EMR) platform preferred.
- Experience with MS Office.
