About The Position

We are seeking an experienced RN Care Manager to support comprehensive patient assessments, care coordination, utilization review, and safe discharge planning. This role works collaboratively with multidisciplinary healthcare teams to identify patient needs, coordinate services, reduce readmissions, and support quality patient outcomes.

Requirements

• Current New York RN license
• BSN or MSN required
• Clinical experience in one or more of the following: Emergency Department, Critical Care, Medical-Surgical Nursing
• Previous Case Management experience
• Experience using Milliman or InterQual criteria
• Strong computer skills

Nice To Haves

• Approximately 3 years of relevant clinical experience
• PRI Certification (must be obtained within 3 months of employment)
• Screen Certification strongly preferred
• Care Management certification, such as CMC or CCM
• Knowledge of healthcare financial, regulatory, and payer issues
• Familiarity with state, local, and federal healthcare programs

Responsibilities

• Perform comprehensive initial and ongoing patient assessments.
• Identify high-risk patients and evaluate discharge planning needs.
• Coordinate multidisciplinary care across inpatient, emergency, surgical outpatient, and inter-hospital settings.
• Develop transitional care plans, including patient and caregiver education and aftercare arrangements.
• Collaborate with healthcare professionals, payers, and other stakeholders.
• Perform utilization review activities, including admission reviews, payer reviews, disposition determinations, and retrospective audits.
