About The Position
We are seeking an experienced Home Care Registered Nurse (RN) to provide high-quality nursing care to adult patients in their homes. The ideal candidate will have strong Med/Surg and Home Care experience with expertise in patient assessment, care planning, and OASIS documentation.
Requirements
- Active New York State RN License.
- AHA BLS Certification.
- Minimum 2 years of recent Med/Surg experience.
- Home Care nursing experience required.
- OASIS knowledge required.
- Valid Driver's License required.
- Every other weekend availability required.
Nice To Haves
- EPIC EMR experience preferred.
Responsibilities
- Assess, plan, implement, and evaluate patient care plans.
- Provide direct nursing care in patients' homes.
- Administer medications and treatments.
- Perform wound care, IV therapy, catheter care, and post-operative care.
- Educate patients and families on treatment plans and home care instructions.
- Complete OASIS assessments and maintain accurate documentation.
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
