The RHTP Primary Care Implementation Specialist serves as a key liaison between Moda Health and partners at rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. This role provides technical assistance, subject matter expertise, and project management oversight to advance value-based payment (VBP) readiness through the National Committee on Quality Assurance (NCQA) Primary Care Medical Home (PCMH) implementation. The position partners with hospital and clinic stakeholders to implement a robust quality improvement and data reporting infrastructure to enable quality measure reporting, support population health management, and prepare clinics for sustainable VBP participation through the NCQA PCMH model. This is a full-time position. The ideal candidate will reside in Alaska’s Anchorage/Kenai Peninsula region. This is a limited duration position contingent on continued receipt of RHTP grant funds.
