The RHTP Grants Management & Implementation Specialist provides grants management, technical assistance, and subject matter expertise to recipients of Moda Health’s Alaska Rural Health Transformation Program (RHTP) grant. This role serves as the primary liaison between Moda Health, funded partners, and other stakeholders to support successful implementation of grant activities. The position is responsible for grant monitoring, compliance, and reporting. This includes ensuring adherence to federal and state requirements and supporting grantees in achieving program goals. The position will also support the development and delivery of training and technical assistance to funded health system partners to implement National Committee on Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) standards and build partner readiness to participate in advanced value-based payment (VBP) models. Working closely with Moda Health’s internal grant and quality teams and external clinical and community partners, the position supports program implementation, reporting to funders, grant proposal development, and continuous quality improvement. This is a full-time position. The ideal candidate will reside within commuting distance of our downtown Portland, Oregon office or in Alaska’s Anchorage/Kenai Peninsula region. Occasional travel to Alaska and a hybrid in-office schedule is required for Portland-based candidates. This is a limited duration position contingent on continued receipt of RHTP grant funds.
