About The Position

The Resource Specialist practices a person centered approach to provide information, assistance, and referral to individuals with disabilities, older adults, and caregivers seeking options and resources. Duties are performed primarily telephonically or via the computer, though occasional contacts may be face-to-face in the office. Uses web based systems to document and track outcomes. Reports directly to Community Services Manager.

Requirements

  • Knowledge of the disability and aging services systems, including long-term support options; consumer-directed services and supports; Medicaid, Medicare, and other benefits.
  • Knowledge of a wide variety of community resources.
  • Skill in empowering, encouraging, and guiding patients and/or caregivers.
  • Skill in active listening, interviewing, and assessing needs.
  • Excellent verbal and written communication skills.
  • Knowledge of HIPAA and ethical standards.
  • Proficient in use of Microsoft Office and use of web based software programs.
  • Excellent organizational and planning skills to efficiently manage time and meet deadlines.
  • Ability to be flexible and adapt to change.
  • Ability to practice good professional judgment and work independently
  • Ability to form rapport with other professionals, patients, and caregivers.
  • Requires a Bachelor’s degree in human services or RN with experience in community based services.
  • Must possess a valid driver’s license, auto insurance, good driving record, and acceptable criminal background check.

Responsibilities

  • Counsels about options in the context of the individual’s needs, preferences, values, and circumstances.
  • Assesses needs, refers, links, follows up, and advocates on clients’ behalf to obtain community resources, as needed.
  • Collaborates with other agencies, health care providers, and community resources to facilitate referrals, as well as to build and maintain professional relationships.
  • Uses web based systems for timely case related documentation, tracking outcomes and data.
  • Plans, prioritizes, and schedules work activities according to urgency of situation, workload, and applicable deadlines.
  • Conducts research to continually identify new resource options. Maintains up to date paper and electronic files on resource information.
  • Educates coworkers and the public on resources and programs available for older adults and individuals with disabilities.
  • Performs additional tasks as required.
