About The Position

This position assists people who are returning to the community after mental health hospitalization and those who are staying in our residential facility to prevent premature hospitalization. Work supports seriously mentally ill (SMI) consumers’ efforts to reintegrate into or remain in their community, thus fostering their success in recovery and in building the confidence and self-reliance needed for full independent living.

Requirements

  • Six months of experience as a Residential Program Trainee; or One year of experience in a Human Services Agency working with the mentally disabled, the elderly, or children and youth; or Two years of college which includes courses appropriate to the Human Services field; or Any equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Build rapport and trust with consumers; work in close contact with consumers in their home environment, an apartment within our residential facility
  • Supports consumers’ efforts consistent with their Individual Service Plan (ISP) in implementing goals, plans, and objectives
  • Teach skills such as budgeting, nutrition, hygiene, laundry, home care, and home safety protocols
  • Documents observations of consumer behaviors and progress in a timely manner and share observations with treatment team members, as appropriate
  • Help arrange for transportation or transport consumer to appointments in a county vehicle when necessary
