About The Position

Located in scenic Honesdale, PA, Wayne County wants you to make a difference every day! We are seeking a compassionate, community-minded professional to join our team as a Supports Coordinator. In this rewarding role, you will help individuals access services, achieve their goals, and actively participate in their communities while working closely with families and support teams. This role supports individuals with intellectual or developmental disabilities and Autism by linking them to helpful services and natural supports.

Requirements

  • Six months of experience as a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or A bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a drug screening.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsibilities

  • Develop and update Individual Support Plans with individuals and their teams
  • Coordinate needed services based on individual needs and goals
  • Encourage individuals to explore community work opportunities when appropriate
  • Complete initial Supports Coordinator training and ongoing learning activities
  • Travel to meet with individuals and teams to review supports and progress
  • Enter required data and maintain organized records
