About The Position

To facilitate client growth and development toward self-sufficiency in daily living skills, socialization, and community living. Facilitate clients' stabilization in the community. Work cooperatively with team members.

Requirements

  • High school diploma or equivalent required
  • Demonstrated crisis intervention/prevention and de-escalation skills required.
  • Proficient communication skills, both oral and written.
  • Ability to work independently and as part of a team.
  • Some knowledge of the mental health system and supports.
  • Valid driver’s license with an acceptable driving record required.

Nice To Haves

  • Associate’s degree preferred in the social sciences or health care-related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.

Responsibilities

  • Facilitate and reinforce skill development in the following areas: social supports, food management, self-care preservation/personal hygiene, relapse prevention, medication management, stress management techniques, money management, utilization of public transportation, and participation in meaningful and planned community activities.
  • Complete documentation, including progress notes and service plans, as required by regulations.
  • Provide resolution and follow-up to crises or problems, in conjunction with managerial input.
