Requirements
- Current Pennsylvania CNA certification required
- Valid driver's license with clean driving record
- Strong communication and organizational skills
Nice To Haves
- Healthcare or long-term care experience preferred
- Comfortable with scheduling/documentation systems
Responsibilities
- Coordinate medical, dental, therapy, and personal appointments
- Schedule and arrange resident transportation
- Escort residents safely to and from appointments
- Work closely with residents, families, care teams, and providers
- Supervise and assist the Transportation Aide
- Maintain appointment and transportation records
- Provide compassionate, resident-centered support
