About The Position

Ohana Pacific Health is seeking a passionate Resident Care Manager / Licensed Practical Nurse to oversee the daily delivery of care for ARCH residents. The role keeps the Director of Clinical informed about progress and potential problems, and coordinates community admissions, discharges, and transfers. The Resident Care Manager plans, develops, and organizes work tasks; coordinates ARCH services and activities in consultation with Case Managers; plans and supervises resident care; continually evaluates residents’ physical conditions; participates in resident care conferences; monitors quality indicators and supports positive outcomes; and participates in admission and discharge planning as the ARCH Case Manager.

Requirements

Graduate of an accredited school of nursing.
Current Licensed Practical Nurse license in the State of Hawaii.
At least three years of experience in a hospital or long-term care facility.
Current and valid two-step TB clearance and health review.

Nice To Haves

Supervisory experience.
First Aid and CPR.

Responsibilities
