About The Position
The Resident Care Coordinator at Avamere Park Place is a full-time position responsible for overseeing resident care, documentation, and staff training. This role requires availability for weekend and on-call coverage, ensuring a high standard of care and a positive living environment for residents. The position is located at Avamere at Park Place in Portland, OR.
Requirements
- Must be at least 18 years of age
- High-School diploma or equivalent
- Must have at least 1 year of Lead Med Tech/RCC experience in senior living
- Must have or obtain First Aid & CPR Certification within 30 days of employment
- Must have or obtain a Food Handlers Card within 30 days of employment
Nice To Haves
- HCA or CNA license preferred
Responsibilities
- Document information pertaining to changes in residents’ status, needs and appointments
- Ensure proper documentation of all care plans, nurses notes and all other resident correspondence
- Provide orientation and on the job training to Caregivers and Medication Aides
- Assist with daily living activities (i.e., bathing, dressing, and eating)
- Measure, record and assist with wellness clinics
- Conduct weekly, monthly, and quarterly audits
- Manage scheduling of clinical staff
- Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy
- Maintain confidentiality of all resident care in accordance with HIPAA guidelines
- Complete on-going training as assigned
- Complete other duties as assigned
