About The Position

The Resident Care Coordinator manages day-to-day resident care operations in Assisted Living while promoting Senior Lifestyle Corporation’s mission and philosophy. The role manages, schedules, evaluates, and supports frontline care staff, and reports to the HWD. The coordinator stays in close contact with resident families, participates in care plan meetings, and addresses care-related service issues. The position pays $36.00 per hour.

Requirements

HPN License.
Study in nursing.
English fluency sufficient to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.
Basic computer skills, including Excel for scheduling.
Ability to be organized, flexible, and even-tempered.
Ability to communicate with families, frontline staff, and the community management team.
Calm demeanor and ability to handle stressful situations.
Ability to multitask, follow through, and work independently.

Nice To Haves

3–5+ years of caregiver experience preferred.
CNA experience preferred.
Experience with scheduling and communicating with families.

Responsibilities

Support the HWD by managing Assisted Living team members.
Schedule Assisted Living team members.
Participate in care plan meetings with residents and families, and manage ISPs as resident needs change.
Promote company policies and ensure communities provide the best possible care for residents.
Participate in weekly at-risk meetings with the HWD, ED, and MCC/MCD to ensure the leadership team is aware of resident needs.
Review and implement processes that support operational efficiency, including shower schedules, laundry schedules, and caregiver assignment sheets.
Maintain close contact with resident families and help address care-related service issues.
