About The Position

Resident Care Coordinators play an important role in ensuring the wellness needs of Ernest P. Barka Assisted Living Community residents are met. They provide personal care treatments and medication administration while working as part of an interdisciplinary team to deliver consistent, high-quality, person-centered care. By building authentic relationships with residents, families, and care partners, they help residents achieve their short- and long-term wellness goals. We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our Resident Care Coordinators are a vital part of our dynamic team!

Requirements

  • Minimum of three (3) years full-time or equivalent experience in a Health Care setting with a minimum of one (1) year in a supervisory role; or An equivalent combination of work and experience will be considered.
  • Licensed nurse in the state of New Hampshire (LPN/RN) required.
  • CPR / Basic Life Support (BLS) certified.
  • Mandatory post offer physical and drug testing for new hire.
  • Criminal records check required.

Responsibilities

  • Provide medication administration, treatments, and personal care while following established care plans, physicians’ orders, and community policies and procedures.
  • Take vital signs, assist with resident admissions and documentation, and initiate individualized personal service plans to proactively manage each resident’s care and services.
  • Communicate with residents, families, physicians, and health service agencies to support positive communication and relationships.
  • Observe and communicate significant changes in a resident’s condition, environment, or other concerns to the Director of Resident Care.
  • Complete required documentation, narcotic counts, shift reports, and monthly MAR accuracy checks while following infection control practices.
  • Help ensure residents are treated with respect and dignity as their healthcare needs change.
  • Participate in Quality Improvement and Risk Management programs, family care conferences, community events, and wellness activities.
  • Encourage teamwork and support coworkers to contribute to a caring, positive environment focused on providing high-quality service to residents.
