About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse to deliver nursing care to residents in a warm, comfortable, home-like environment. The role works as part of a supportive team and helps foster community and lasting relationships with residents and associates. The position also involves coordinating resident care, supervising Resident Care Assistants, and supporting residents’ independence and well-being.

Requirements

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program, as required by state requirements.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, as required by state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their work.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands with support for resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources per state regulations.
• Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
