About The Position

The Care Coordinator Nurse at Lankenau Medical Center coordinates patients’ care across the healthcare continuum. The nurse works with physicians, peers, community colleagues, patients, and families to identify opportunities to improve patient care experiences and services, population health, cost effectiveness, and collaborative practices. The role partners with patients, families, physicians, and colleagues to develop, implement, and monitor comprehensive plans of care that include health promotion, disease prevention, and disease management activities. The position is per diem, with varied shifts mainly on weekends and some weekdays for coverage, requiring 16 hours per month, one weekend per month, and one holiday per year.

Requirements

• Three years of recent clinical experience.
• An Associate degree or BSN upon hire; nurses hired with an Associate degree must earn a BSN within three years of hire.
• Active Pennsylvania RN license.
• Availability for 16 hours per month, one weekend per month, and one holiday per year.

Responsibilities

• Coordinate patients’ care across the healthcare continuum.

• Partner with patients, families, physicians, and colleagues to develop, implement, and monitor comprehensive plans of care.
• Include health promotion, disease prevention, and disease management activities in care plans.
• Participate in hospital and system-wide Nursing Committees, Unit Councils, and evidence-based research projects.
