About The Position

The Remote Triage Nurse provides after-hours clinical decision support for senior living communities when on-site staff are limited, ensuring timely and appropriate guidance and escalation. The position is fully remote and scheduled for evenings or overnights, including every other weekend.

At the start of a shift, the nurse signs into TriaTech and handles after-hours calls from frontline caregivers, family members, hospice partners, and other clinicians. The nurse conducts phone-based clinical triage and assessment, clarifies next steps, coordinates with community staff and external providers, and documents each interaction in TriaTech and the appropriate EHR. Notes must be focused and complete to support continuity for the morning team, with records finalized and ready for follow-up at handoff.

Requirements

• Active RN license.
• At least 5 years of hands-on senior care experience in Assisted Living, TCU, or LTC.
• Strong clinical judgment and rapid prioritization skills.
• Calm communication under pressure.
• Proficiency with electronic documentation and comfort learning new platforms.

Responsibilities
