About The Position

BAYADA Home Health Care is seeking a Registered Nurse to join its home health visit team in the Upper Montgomery County area. The nurse is an integral member of a multidisciplinary health care team providing skilled nursing and rehabilitative care to clients so they can remain at home. The role involves delivering compassionate, high-quality care in varied home environments, working independently, making real-time clinical decisions, and responding to changing patient needs. Full-time, part-time, and per-diem opportunities are available, with flexible scheduling and one-to-one care.

Requirements

• Hold a current nursing license in good standing in Pennsylvania.
• Graduate from an accredited and approved nursing program, as evidenced by a school transcript or diploma.
• Have at least one year of current or recent hands-on medical experience within the past two years with adult or geriatric patients in an acute care, emergency department, home health, hospice, ICU, med-surg, sub-acute care, or rehabilitation unit in a medical or skilled nursing setting.
• Work independently while recognizing when to escalate concerns.
• Make real-time clinical decisions without immediate on-site support.
• Demonstrate strong problem-solving skills in unpredictable home environments.
• Pay close attention to detail while maintaining productivity expectations.
• Adapt to changing patient needs and schedules, and demonstrate cultural sensitivity and respect for patient autonomy.

Responsibilities

• Perform strong head-to-toe assessments and identify subtle changes in patient condition.
• Provide care for acute and post-acute needs, including CHF, COPD, wound care, IV therapy, and post-surgical care.
• Triage and independently manage urgent clinical situations in the home setting.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Conduct comprehensive wound assessments, provide wound care, and document findings.
• Prioritize visits and manage a dynamic visit schedule, including urgent visits.
• Use point-of-care documentation and technology efficiently.
