About The Position

PruittHealth @ Home is hiring a compassionate Registered Nurse (RN) to provide home health care across York County, South Carolina, with a main office in Rock Hill. The RN provides autonomous, one-on-one patient care in patients’ homes while working with an interdisciplinary care team. Requirements

• Active, unrestricted Registered Nurse (RN) license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• At least 1 year of RN experience.

Nice To Haves

• Home health or acute care experience is preferred.
• Experience with aging or adult patient populations.
• OASIS experience is a plus.
• Strong organization and time-management skills, including the ability to multitask and prioritize a busy caseload.
• Confidence working with technology and electronic documentation systems.

Responsibilities

• Provide skilled nursing visits in patients’ homes.
• Complete patient assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assistance.
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.
