About The Position

Using the principles of person-centered care, the registered nurse is responsible for providing services to referred stroke clients in a variety of settings, with a focus on symptom management and stroke prevention. The nurse provides client-specific consultation and education, training and in-services for service providers, caregivers and families.

Requirements

Registered member in good standing of the College of Nurses of Ontario
Current Basic Life Support Certification
Valid Ontario Driver’s License
Clinical expertise and experience in stroke rehabilitation and demonstrated knowledge of current stroke rehabilitation best practices
Knowledge of community service providers within Huron and Perth counties
Understanding of Canadian Stroke Best Practices
Demonstrated knowledge of current evidence and effective strategies for vascular risk factor monitoring and management and secondary stroke prevention
Working knowledge of and experience with theories and principles guiding adult learning and patient self-management approaches
Knowledge of the principles of adult education
Sound understanding of the role of a variety of rehabilitation professionals in the provision of stroke rehabilitation
Excellent assessment skills and development of personalized patient informed treatment plans
Demonstrated ability to prioritize and function both independently and collaboratively with the inter-professional care team
Knowledge and skill caring for stroke patients and individuals with complex medical needs
Demonstrated commitment to ongoing professional development
Well-developed interpersonal, communication and teaching skills
Ability to plan, think critically, organize, and problem solve
Strong interpersonal skills with a demonstrated ability to establish positive relationships with clients, families, caregivers and community services
Excellent English verbal and written communication skills using different mediums, such as telephone, in-person and video
Evidence of continuing education and professional development
Proficiency with computer applications including Microsoft applications and working knowledge of or the ability to quickly comprehend the Meditech Information System

Nice To Haves

Completion of Canadian Hemispheres course preferred
Experience in both hospital and community service delivery is an asset
