About The Position
The Arc of San Diego is hiring passionate Nurses to provide care to clients with intellectual and developmental disabilities in their own homes. The Arc of San Diego operates nine (9) ICF-DD-H residential facilities across San Diego County. We believe in putting people first and creating meaningful days for our clients and each other. This position will support adults with intellectual and developmental disabilities and will be responsible for 4 residential homes throughout San Diego County.
Requirements
- Active CA license as Registered Nurse
- 6 months hands-on experience working with people with intellectual and/or developmental disabilities
- Valid CA Driver’s License with clean driving record
Nice To Haves
- Experience using electronic case management systems (Therap)
- Previously provided in-home care in individual or group setting
- Experience working with people with intellectual and/or developmental disabilities
Responsibilities
- Provide care to clients with intellectual and developmental disabilities in their own home.
- Provide full care coordination.
- Act as a strong client advocate.
- Observe the rights and best interest of adults with intellectual/developmental disabilities.
- Perform physical and nutrition assessments.
- Develop individual health care plans.
- Provide case management support.
- Oversee clients' specialty appointments.
- Conduct pharmacy and medication review.
- Conduct psychiatrist and mental health review.
- Schedule and attend coordinated client service planning meetings.
