About The Position

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide compassionate, individualized hospice care wherever patients call home, including private residences, skilled nursing facilities, and assisted living communities. The nurse collaborates with an interdisciplinary team of nurses, social workers, chaplains, CNAs, and volunteers to develop care plans that address patients’ and families’ physical, emotional, and spiritual needs. Lower clinician caseloads and higher staff-to-patient ratios support meaningful time with patients and families and personalized care. Various nursing opportunities are available based on the nurse’s strengths and interests.

Requirements

• Current Registered Nurse (RN) license in good standing in the state of practice.
• Passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently while effectively managing a daily schedule.
• Excellent time-management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and using electronic medical record (EMR) systems.
• Reliable transportation, a valid driver’s license, and current auto insurance for travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care in patients’ homes, skilled nursing facilities, and assisted living communities.
• Collaborate with nurses, social workers, chaplains, CNAs, and volunteers to develop care plans addressing patients’ and families’ physical, emotional, and spiritual needs.
• Manage a daily schedule that balances patient visits, documentation, meetings, and urgent needs.
• Travel throughout the service area to visit patients.
