Hospice RN Sat/Sun - Stamford/Greenwich, CT
About The Position
Constellation Health Services is seeking a Registered Nurse with hospice experience to provide Saturday and Sunday coverage for patients at Greenwich Hospital, Stamford Hospital, and Norwalk Hospital. Shifts are eight or ten hours and are non-Baylor. The position is based in Greenwich, Connecticut, with travel of up to 25%. A valid Connecticut nursing license is required for consideration. The nurse provides skilled care to patients in their home setting, including medications and treatments prescribed in the physician’s plan of care. The role includes comprehensive assessment, care coordination, monitoring and reporting changes in patient status, participation in weekly interdisciplinary team meetings, and timely completion of clinical documentation. Weekend and holiday coverage may also be needed.
Requirements
- Graduate of an accredited RN program.
- Registered Nurse license in the state of employment; a valid Connecticut license is required for consideration.
- At least 1–2 years of RN experience.
- Hospice RN experience is required.
Nice To Haves
- Liaison experience is highly desired.
- Previous hospice care experience is strongly desired.
- Basic knowledge of the iOS operating system.
Responsibilities
- Provide skilled nursing care in patients’ homes, including administering medications and treatments prescribed in the physician’s plan of care.
- Perform comprehensive assessments and coordinate the plan of care with relevant disciplines, family members, and primary caregivers.
- Observe and evaluate patients’ physical and emotional status and report changes to the nursing supervisor and attending physician.
- Participate in weekly Interdisciplinary Team Meetings and provide updates on patients’ physical, emotional, psychosocial, and spiritual status.
- Maintain and process documentation in compliance with agency standards.
- Use a computer-based documentation system and submit clinical documentation, time records, and other required information within required time frames.
- Provide input to the Home Health Aide Supervisor as needed for hospice aide evaluations.
- Provide weekend and holiday coverage as needed.
