About The Position

Elara Caring is hiring a full-time Home Health Baylor Registered Nurse in New Haven. The RN provides personalized, one-on-one care to help patients recover, manage chronic conditions, and remain independent at home. The nurse manages a dedicated caseload, organizes a field schedule independently, and receives support from an experienced clinical team. The schedule is Saturday through Monday, with 12-hour shifts.

Requirements

• Current, unrestricted RN license.
• Associate Degree in Nursing required.
• At least one year of clinical nursing experience.
• Home health experience required.
• Strong assessment, communication, and care-coordination skills.
• Valid driver’s license, dependable transportation, and auto insurance.
• Ability to travel throughout the assigned territory.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Homecare Homebase experience.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders.
• Monitor patient conditions, medications, and treatment progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations and emergency room visits.
