About The Position
As a PRN Registered Nurse, you will coordinate and oversee patients’ home care while collaborating with care teams to develop and implement individualized care plans. You will assess patients, provide skilled nursing care, educate families, maintain timely documentation, and communicate progress to the clinical manager, physician, and care team. Requirements
- Current RN license specific to the state(s) assigned.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and liability insurance.
- Home Health experience.

Responsibilities
- Coordinate and oversee all aspects of patients’ home care.
- Collaborate with patient care teams to develop and implement care plans.
- Educate families on the patient’s condition, needs, and care.
- Perform head-to-toe physical assessments.
- Check vital signs and manage medications.
- Give injections, provide IV therapy, or administer tube feedings.
- Administer and maintain catheters.
- Complete documentation accurately and on time.
- Supervise LPNs and HHAs.
- Regularly communicate patient progress to the clinical manager, physician, and care team.
- Perform other duties as assigned.
