The Care Team registered nurse works in the Department of Family Medicine, Community Internal Medicine, and Community Pediatric and Adolescent Medicine Primary Care. The RN coordinates nursing care, including direct patient care, patient and family education, and transitions of care. The role includes visit and non-visit interactions with patients and supporting care providers and requires communication and collaboration with providers and other Care Team members. Primary Care services include care for existing medical needs, preventive and acute care, and management of chronic health conditions. Care is provided through provider or nurse visits, telehealth, and web-based care. Primary Care locations are fast-paced, team-oriented environments in which RNs work closely with an interdisciplinary team and build long-term relationships with patients and their care providers. The RN supports professional nursing practice across settings and the continuum of care, functioning within the Mayo Clinic Nursing Professional Practice Model and the ANA Nursing: Scope and Standards of Practice. The RN plans, implements, evaluates, and communicates all phases of nursing care for assigned patients; provides leadership through precepting, formal and informal leadership, and quality-improvement efforts; and delegates care according to skill, experience, patient acuity, fiscal accountability, and available resources. The RN also mentors and teaches, communicates effectively, and may participate on committees or projects. New graduate nurses are directed to apply through the Nurse Residency Program. This role is eligible for TN sponsorship; successful sponsorship requires state licensure and completion of VisaScreen or equivalent certification.
