Join a compassionate, collaborative Movement Disorders team dedicated to supporting patients with complex neurological conditions, throughout their long-term care journey. As an LPN, you'll play a meaningful role in building lasting relationships with patients while providing clinical support that helps them navigate treatment, manage symptoms, and coordinate their ongoing care. This is an ideal opportunity for an LPN who enjoys patient education, relationship building, and making a lasting impact in an outpatient specialty setting. In this role, you'll support physicians and registered nurses by rooming patients, assisting with clinic visits, coordinating care, responding to MyChart messages, triaging patient concerns within the LPN scope of practice, and helping facilitate prior authorizations, medication refills, and disability paperwork. You'll work closely with patients, providers and interdisciplinary team members to ensure a seamless care experience while helping patients access the treatments and resources they need. The ideal candidate is a compassionate, organized, and patient-focused LPN who thrives in a fast-paced ambulatory environment. Experience in neurology, chronic disease management, ambulatory care, case management support is preferred, along with strong communication, critical thinking, and teamwork skills.
