About The Position

Adoration Health is seeking a dedicated Home Health Registered Nurse to provide patient-centered care in Lancaster, South Carolina. The role serves Lancaster and Chester counties and involves working independently while receiving support from a team. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with participation in an on-call rotation as required by the local branch.

The nurse assesses patients’ physical, emotional, and psychological needs; develops care plans aligned with physician orders and patient goals; provides nursing care and symptom management; and supports patients’ families and caregivers. The role also includes collaboration with an interdisciplinary team and accurate, timely documentation.

Requirements

• Associate or bachelor’s degree in nursing from an accredited college of nursing.
• Current unrestricted registration and nursing license in the state of practice and state of residence.
• At least one year of nursing practice in a patient care setting.
• Valid driver’s license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
• Current CPR certification.

Nice To Haves

• Home health, geriatrics, or other related-setting experience is preferred.
• New nursing graduates may be considered in select markets based on program availability.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create home health care plans that align with physician orders and the patient’s goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
