Registered Nurse - Hospice Admissions - San Luis Obispo
About The Position
Central Coast Home Health and Hospice is seeking a Per Diem Hospice Admit Nurse to join its Hospice team in San Luis Obispo. The Admit Nurse completes initial patient assessments to determine hospice needs, counsels patients and families, and provides education related to their needs. The role calls for accuracy and flexibility. Nurses provide high-quality care to hospice patients in their homes and collaborate with an interdisciplinary team to promote patient and family choice, ensure continuity of care, and enhance the quality of life of dying patients. The position requires organization and the ability to work independently, manage a caseload, and set a work schedule. The schedule is Fridays and one weekend day, Saturday or Sunday, from 8 AM to 5 PM, with flexibility. There is no night call.
Requirements
- Current California RN license.
- At least one year of professional nursing experience.
- CPR certification for Healthcare Providers.
- Reliable vehicle for work-related driving.
- Current driver’s license and clean driving record.
- Maintain auto insurance coverage.
- Accuracy, flexibility, organization, and ability to work independently.
Nice To Haves
- Hospice and/or home health experience.
- Understanding of hospice philosophy and issues of death and dying.
