About The Position

Amedisys is hiring a full-time hospice admissions registered nurse for the first shift. The nurse coordinates hospice patient admissions and assesses patients’ initial physical, psychosocial, and spiritual needs. They help establish the initial plan of care with the attending physician, medical director, and interdisciplinary team; provide skilled interventions and instructions to support symptom control and comfort; and serve as a liaison among disciplines and facilities during admission. The role includes maintaining accurate and timely patient care records, communicating significant findings and changes to the care team and physician, obtaining physician orders as needed, and performing on-call responsibilities as assigned.

Requirements

• Current RN license specific to the state or states assigned.
• One year of clinical RN experience.
• Current CPR certification.

Responsibilities

• Coordinate hospice patient admissions and assess patients’ initial physical, psychosocial, and spiritual needs.
• Coordinate establishment of the initial plan of care with the attending physician, medical director, and interdisciplinary team.
• Provide skilled interventions and instructions to patients and caregivers to support symptom control and patient comfort under physician direction.
• Act as a liaison between disciplines and facilities during the admission process.
• Develop, prepare, and maintain individualized patient care progress records accurately and promptly, following policies; submit accurate documentation and record pain or symptom management changes and outcomes as appropriate.
• Communicate significant findings, problems, and changes in the patient’s condition or environment to the clinical manager, physician, and other hospice care team members.
• Communicate with physicians about patient needs, report changes in condition, and obtain physician orders as required.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
