About The Position

The Registered Nurse (RN) Field Admissions Specialist completes hospice admissions and patient visits as needed. The role involves orienting new patients to the hospice program, assisting patients and caregivers with legal paperwork, and completing thorough nursing assessments. The position requires working four 10-hour shifts per week, Monday through Thursday, from 12:00 p.m. to 10:30 p.m., and traveling throughout the Bay Area, up to 150 miles per shift. Patients may be seen in private homes, assisted living facilities, hospitals, and other places where they live or receive care.

Requirements

• At least 1 year of experience in hospice, a skilled nursing facility (SNF), or a hospital.
• Willingness and comfort with regular long-distance travel throughout the Bay Area, up to 150 miles per shift.
• Reliable transportation, a valid driver’s license, and auto insurance.
• Registered Nursing licensure in the state of California.
• Willingness to travel to patients in private homes, assisted living facilities, hospitals, and other settings.

Responsibilities

• Complete hospice admissions and patient visits as needed.
• Orient new patients to the hospice program.
• Assist patients and caregivers with required legal paperwork.
• Complete thorough nursing assessments.
• Establish hospice eligibility and diagnosis when the patient is appropriate.
