About The Position

Transitions Hospice is hiring an RN to serve as a flexible clinical support member of its interdisciplinary care team across Northern Illinois. The nurse steps in across cases to assess and treat patients, address patient and family needs, and communicate with the care team. The role requires adaptability and confidence working in varied environments and assignments. The schedule is 8 AM–8 PM, with alternating four-day and three-day workweeks. Weekends are included in rotation, along with rotating on-call and holiday coverage. The listed annual pay range is $85,000–$95,000.

Requirements

Active RN license in the state of employment.
Graduate of an accredited school of Nursing.
Flexibility and confidence working across varied patient assignments.
Strong assessment skills and sound clinical judgment under pressure.
Excellent documentation habits and communication skills.
Valid driver's license and willingness to travel locally.

Clinical experience in hospice, home health, or a related setting is preferred.

Responsibilities

Deliver professional nursing care across the full nursing process within the scope of practice.
Conduct patient and family assessments and contribute to individualized care plans.
Complete visit types as needed to meet patient and family needs.
Administer medications and treatments according to physician orders.
Identify nursing diagnoses based on health assessment data and respond accordingly.
Record accurate and timely clinical notes, including pain and symptom management outcomes.
Communicate changes in patient condition to physicians and obtain orders as required.
Coordinate with community health partners to support care transitions.
Arrange necessary equipment and services for patients.
Maintain and develop clinical skills through continuing education and in-service participation.
